Hartford Life Open Enrollment 2010

 

Dear Employee,

 

             The North American Division (“NAD”) Hartford Life and AD&D Enrollment will occur from March 1 through April 15, 2010.  The effective date for benefits will be June 1, 2010 (unless medical evidence is required).  Applications must be signed and returned to your Human Resources Representative as early as March 1st and no later than April 15th.

 

             Now is a great time to buy Supplemental Life Insurance that provides you with additional financial protection for life’s unexpected events.

 

Why is this a good time to enroll?

 

             You have the opportunity to buy life insurance at economical group rates.  Additionally, you have the opportunity to buy a guaranteed amount of life insurance without showing evidence of good health if you enroll during this enrollment period.  The option to elect Accidental Death and Dismemberment (AD&D) coverage also will be available.  The coverage (AD&D) is not subject to medical underwriting.

 

Here are the rules to follow:  

 

1. If you choose to participate in this open enrollment, please be sure to have current information about your salary, the amount of supplemental life insurance and the amount of accidental death and dismemberment coverage you now have.

 

2. Supplemental life elections that do NOT require evidence of good health:

 

New entrants and current enrollees may elect the lesser of 3x

salary (rounded down to the lower $10,000 increment) or $250,000.

 

Employees currently at or over the guarantee issue (“GI”) amount of $250,000 may increase by $10,000.

 

Spouses may elect $30,000 (but not to exceed 100% of the

employee coverage).

 

Spouses currently at or over the GI level of $30,000, may increase by

$10,000 (but not to exceed 100% of the employee coverage).

 

Child coverage may be elected in increments of $1,000 up to $25,000.

 

             If your election is in excess of the guarantee issue, please contact your human resources office to obtain a personal health application.   The amounts in excess of the GI must be approved by The Hartford.  

 

3.  If you are not making any changes to your existing coverage or you are declining to participate in this open enrollment, no action is required.

             f.  Personal Health Statement Application(if requesting an insurance amount beyond the              prescribed limits of the open enrollment)

 

 

Applications for employees participating in this open enrollment must be signed and returned to your Human Resources Representative as early as March 1st and no later than April 15th.
